Cardiology consultation

Heart failure with reduced ejection fraction (EF 30%, normal >50%), likely due to ischaemic heart disease. Screen for non-ischaemic causes of heart failure, e.g. thyroid disease, iron studies.

Pharmacotherapy: review antihypertensive treatment, review lipid lowering treatment, commence diuretic. Fluid restriction recommended, with daily weight monitoring and titration of diuretic pharmacotherapy.

Lifestyle: Cardiac rehabilitation program for regular exercise. Consider dietitian referral for low-sodium, low-lipid diet monitoring. Smoking cessation counselling, alcohol reduction/cessation.

Ensure patient is aware of importance of adherence to treatment.
