I – Hello, my name is ___ and I am a JMO.

S – I have a 52 year-old man, Bruce Adamski, with suspected acute myocardial infarction and peripheral hypoperfusion. He presented to ED with central chest pain and dizziness which started 45 minutes ago.

B – He has a history of hypertension and dyslipidaemia, previous occurrences of chest pain on exertion as well as family history of stroke and acute coronary syndrome. Former smoker, approximately 15 pack/years, consumption of 14 standard drinks per week. On examination he is hypotensive with prolonged capillary refill, diaphoretic, and appears in discomfort. An ECG shows ST elevation in anterolateral leads. Troponin level of 225ng/L (elevated).

A – I believe this patient is having a STEMI, with risk of cardiogenic shock.

R – I recommend administering aspirin, and pain relief, cardiac monitoring. Repeat troponin and ECG, call an urgent Cardiology consult for reperfusion therapy.

Needed to win

Need to include all 5 sections of ISBAR, plus mention at least four of the following:

  1. Central chest pain and dizziness, sudden onset 45 minutes ago.
  2. At least 4 of the following 6: hypertension, high cholesterol, chest pain on exertion, family history of vascular disease, smoking history, drinking history.
  3. ECG shows ST elevation in leads V2-V6, I and aVL (anterolateral).
  4. Elevated Troponin level.
  5. Cardiology consult, PCI indicated.
