Cardiology consultation

Paroxysmal atrial fibrillation. If not conducted, echocardiogram needed to evaluate valvular and atrial structure, plus to investigate pre-existing atrial thrombus. Thyroid function tests, UEC to evaluate potential causes. CHA₂DS₂-VASc 1, consider anticoagulation, or 3 if existing thrombus, anticoagulation indicated. Given smoking history, anticoagulation recommended. Beta blockers indicated for rate control. Smoking cessation, alcohol reduction counselling.
